NHS Health Checks privacy notice

About the service

This service is managed by the council’s Public Health Team and is delivered on our behalf by GP practices in Barnet. It is part of the national NHS Health Checks programme to identify whether people have a higher risk of health problems like heart disease, diabetes, kidney disease or stroke (NHS Health Check - NHS (www.nhs.uk)).

You are eligible for an NHS Health Check under the Barnet service if you are:

·       aged 40-74

·       do not already have any of the health problems that are looked for during an NHS Health Check

·       registered with a Barnet GP practice that delivers the service.

We offer this service because the Health and Social Care Act 2012 section 12 places a duty on local authorities to take such steps as it considers appropriate for improving the health of the people in its area by:

  • providing information and advice
  • providing services or facilities designed to promote healthy living. Whether by helping individuals to address behaviour that is detrimental to health or in any other way.

NHS Health Checks are provided as part of this duty.

The Local Authorities (Public Health Functions and Entry to Premises by Local Healthwatch representative) Regulations 2013 place a duty on local authorities to provide NHS Health Checks to eligible people in their area.

The Health and Social Care Act also allows us to use personal information for research purposes and this covers the collection and analysis of NHS Health Check data by the council.

Your NHS Health Check will be carried out by staff at your GP practice and recorded on their patient database. They will share information about your NHS Health Check with the council’s Public Health Team. The information is pseudonymised (it does not include your name, date of birth or full address).

Personal information collected.

The council collects the following information from GP practices:

  • first part of postcode, LSOA and MSOA
  • age
  • equalities information
  • health or medical information  

Who we share the information with

The service sends quarterly performance reports to NHS Health Check Official Statistics, which do not contain any identifiable information about individuals.
